It's actually Juliet, who said the quote directly, but it comes from the play.

With the other two quotes John Proctor says that before he is about to be hanged for witchcraft. "Because it is my name" They are asking to rebuke his name and confess, His honest won't allow him to do so.

The last quote is when Inigo Montoya and the Dread Pirates Roberts are fighting amongst the Cliff of Insanity (dueling I might clarify) and he is wearing a mask so his true identity is not known.

So, it's a mixture of names really. A mixture of actions and feelings.
